- 1 500 mL sample collection bottle
- 2 100 mL sample collection bottles
- 1 Pair of nitrile gloves
- A Chain of Custody form to fill out when sending your sample back to our lab.
*Please consult your lender before purchasing a test. In some instances, a licensed third-party sample collector is required.
